Center for Human Development (CHD) seeks a Per Diem Licensed Practical Nurse (LPN) to join the Heron House EMGLE program in East Longmeadow, MA. The role supports a small LPN-to-client ratio under supervision of charge nurse or RN, with a dedicated care team and rotating on-call responsibilities.
The LPN will perform medication administration, participate in care plan meetings, provide direct care, monitor vital signs, and support activities of daily living.
Center for Human Development, (CHD), is currently looking for Per Diem Behavioral Health Licensed Practical Nurses (LPN)to join the team at Heron House, an Enhanced Medical Group Living Environment (EMGLE) program. This 18-bed facility located in East Longmeadow, MA allows you to provide quality care and support to those we serve with a small LPN to client ratio. Under the supervision of designated charge nurse or RN, you will work with a support team of LPN’s, CNA’s and Direct Care staff providing care and compassion all the while gaining valuable experience in CHD’s modern EMGLE facility.
The Enhanced Medical GLE (EMGLE) provides a variety of skilled healthcare and supportive services including around the clock nursing and personal care assistance to Persons with mental illness in addition to compounding medical needs. These services are designed to meet and support the daily needs of Persons with chronic medical conditions, terminal illnesses and/or disabilities, which are impacted by their significant mental illness. These Persons require licensed and unlicensed professional services to meet their chronic complex medical and behavioral needs to ensure improvement or stabilization of their medical and behavioral condition. This model will also serve to maintain, prevent, or slow worsening of condition as well as assisting transition to a lower level of care setting or prevent hospitalization/nursing facility admissions.
A competitive hourly wage of $38.50/hr.With a $2.00/hr. differential for overnight shifts.
Schedule: Per Diem - all shifts open for per diem.
