Job summary

NursePower seeks an experienced Registered Nurse (RN) to provide per diem, private duty home care in New London County, CT. You will deliver one-on-one nursing care in clients’ homes and coordinate with case managers to ensure quality outcomes.

The role offers flexible weekday hours, mileage reimbursement, and a supportive nurse-led team. CT license and 3+ years of home health experience are required.

Qualifications

• Active, unencumbered RN license in Connecticut.
• Minimum 3 years of home health or private duty nursing.
• Valid driver's license and reliable personal vehicle.

Responsibilities

• Conduct home visits and administer medications per care plan.
• Follow care plans and coordinate with Nurse Case Manager.
• Record vital signs and report deviations.
• Advocate for changes in care and arrange additional hours as needed.
• Educate clients and families on dietary and care protocols.
• Communicate with physicians and families about health status.
• Provide safe transportation and assist with appointments.
• Complete timely clinical documentation.

What You’ll Do

• Conduct home visits to assess client needs, administer prescribed medications, perform wound care, and manage medical devices.
• Follow individualized care plans and collaborate directly with the Nurse Case Manager before making necessary adjustments.
• Measure, record, and report any deviations from a client’s baseline vital signs.
• Identify changing health needs, advocate for increased care services or hours, and coordinate implementation with the client's responsible party.
• Educate clients and families on dietary recommendations, nutritional goals, and care protocols.
• Communicate proactively with physicians and family members regarding health status, concerns, and clinical updates.
• Provide safe client transportation and advocacy for required medical appointments as needed.
• Complete, accurate, timely clinical documentation that reflects high professional standards.

Qualifications

• Required License: Active, unencumbered Registered Nurse (RN) license in the State of Connecticut.
• Required Experience: Minimum of 3 years of home health, private duty, or community-based nursing experience.
• Vehicle & Travel: Valid driver’s license, reliable personal vehicle, and active auto insurance.
• Core Competencies: Strong independent clinical judgment, acute observational skills, and strong initiative.
• Physical & Background: Ability to travel throughout New London County, assist with client transfers as needed, and satisfy all state background and health onboarding requirements.
