Job summary

NursePower is seeking a per diem Registered Nurse (RN) in Connecticut to provide high-quality home health care to clients in private homes and assisted living communities. You will conduct home visits, administer medications, and collaborate with the Nurse Case Manager to adjust plans as needed.

This flexible role offers competitive hourly pay, mileage reimbursement, and opportunities for growth within a nurse-led management team, with travel across New London County to support clients in need.

Qualifications

• Active, unencumbered Registered Nurse (RN) license in Connecticut.
• Minimum of 3 years of home health, private duty, or community-based nursing experience.
• Valid driver’s license, reliable personal vehicle, and auto insurance.
• Strong independent clinical judgment, acute observational skills, and initiative.
• Ability to travel throughout New London County and satisfy onboarding requirements.

Responsibilities

• Conduct home visits to assess client needs and administer medications.
• Follow care plans and collaborate with the Nurse Case Manager.
• Measure and report deviations from baseline vital signs.
• Identify changing health needs and coordinate with the client’s responsible party.
• Educate clients and families on dietary recommendations and care protocols.
• Communicate with physicians and family about health status and updates.
• Provide safe client transportation and advocacy for appointments as needed.
• Complete timely clinical documentation reflecting high professional standards.
