Job summary

FellowshipLIFE in Lakewood, NJ is seeking a compassionate Physical Therapist Assistant to join our Rehabilitation team on a per diem basis. You will support licensed PTs, implement treatment plans, and help residents maintain mobility and independence.

The role requires graduation from an accredited PTA program (APTA-recognized) and a current New Jersey PTA license. Strong communication and teamwork are essential.

Qualifications

• Graduate of an accredited PTA program (APTA-recognized).
• Current New Jersey Physical Therapist Assistant license required.
• Strong interpersonal and communication skills.
• Ability to work effectively within a multidisciplinary healthcare team.

Responsibilities

• Implement resident-specific treatment plans developed by the supervising Physical Therapist.
• Assist Physical Therapists with evaluations, assessments, tests, and therapeutic procedures.
• Guide residents through mobility activities and therapeutic exercises designed to improve strength, balance, endurance, and functional independence.
• Collaborate with the rehabilitation team to deliver individualized care plans that meet each resident's unique needs and goals.
• Monitor resident progress and communicate updates to the supervising Physical Therapist.
• Complete clinical documentation accurately and in a timely manner.
• Participate in care plan meetings and interdisciplinary team discussions as directed by the Director of Rehabilitation or Office Manager.
• Maintain therapy equipment and report any repair or maintenance needs.
• Provide exceptional customer service to residents and families in a compassionate and professional manner.
