Job summary

Aviara Healthcare Center is seeking a Staff Physical Therapist Assistant to assist the Physical Therapist with resident care and documentation. You will maintain safety, educate residents and families, and participate in rehab meetings while following California PTA guidelines.

The role requires an Associates degree from an accredited PTA program and active state licensure, with proficiency in Net Health and Point Click Care.

Qualifications

• Associate degree from an accredited PTA program.
• State PTA licensure required; continuing education per state rules.

Responsibilities

• Treat residents as directed by Physical Therapist.
• Complete timely, accurate documentation per facility and payer requirements.
• Maintain a safe treatment environment with infection control practices.
• Assist in conferences and rehab meetings; comply with California laws for PTAs.
• Train Restorative Aides in the department as needed.
• Communicate progress and discharge planning with the supervising PT and care team.
• Educate residents and families on goals, home programs, safety, and fall prevention.
• Assist with ordering durable medical equipment to promote independence.
• Maintain resident and company confidentiality; report equipment issues.
• Contribute to QA and rehab program development; collaborate with staff.
