Job summary

Sigma Health Rehab in Bath, NY is actively seeking a per diem Physical Therapy Assistant (PTA) to support residents in a Skilled Nursing Facility. You will work under supervision to carry out the plan of care and help residents regain daily function.

Qualifications

Graduate of an accredited PTA Program.
Current New York State PTA License.

Responsibilities

Select therapy activities that follow the plan of care set out by the supervising occupational therapist.
Assist residents in carrying out daily activities.
Monitor residents' performance in therapy activities, provide encouragement and document on residents as needed.
Teach residents how to use special equipment to assist with daily function.
Collaborate with the PT to establish a rehabilitation plan.
Maintains a cooperative relationship among healthcare team members & contributes to team effort.
