Seabrook Village by Erickson Senior Living is hiring a per diem Physical Therapist to support our Home Care Department serving residents within the Seabrook community in Tinton Falls, NJ. You will implement personal care plans, emphasizing residents' strengths and routines while delivering holistic PT services.

Join an interdisciplinary team, with onsite physicians and wellness resources, to support ongoing mobility, safety, and quality of life for older adults in a continuing care retirement

Qualifications

• Geriatric physical therapy experience is preferred.
• Home care experience preferred for residents living within the community.
• Experience with OASIS is a plus.

Responsibilities

• Implement residents' personal care plans focusing on their strengths and routines.
• Provide holistic PT services addressing physical, spiritual, emotional, and clinical needs.
• Collaborate with interdisciplinary team to meet residents' care goals.
