Job summary

Seabrook Village by Erickson Senior Living is hiring a per diem Physical Therapist to join our residential care team. You will deliver on-site PT services within a thriving campus, focusing on residents’ goals and comfort while adhering to care plans and regulatory requirements.

The role emphasizes holistic wellness for older adults, collaboration with an interdisciplinary team, and flexible hours with per diem compensation. Regulation-compliant documentation and state licensure are required.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• Experience with older adults across multiple diagnostic groups is preferred.
• Experience with home care setting is a plus.
• Experience with OASIS is a plus.

Responsibilities

• Provide physical therapy services to residents within the gated community.
• Implement residents' personal care plans focusing on strengths and routines.
• Collaborate with interdisciplinary clinical team to address resident needs.
• Complete documentation to meet reimbursement and regulatory requirements.

We are hiring a resident-centered Physical Therapist to join our team. All of our residents live within our award-winning campus, creating an environment where you can be part of their journey throughout their continuum of care. Our Physical Therapists provide a holistic approach, supporting the residents' physical, spiritual, emotional, and clinical needs. They are responsible for implementing residents’ personal care plans, focusing on residents’ strengths, preferences, and preferred routines, while providing a sense of comfort, companionship, and belonging.

Our communities offer levels of care and include specialty programs such as: urinary incontinence, vestibular rehab, home safety assessment, movement disorders and low vision.

What we require:

• Must possess a valid and current state Physical Therapy license. Degree and training must meet current Medicare and state requirements.
• The ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.
• Experience with seniors in home care setting preferred
• Experience with OASIS is a plus
