Qualifications

• Master's Degree in Social Work required.
• Licensed Social Worker in PA preferred.
• Minimum of 1 year inpatient psychiatric unit experience with crisis intervention.
• Ability to assess child, adolescent, adult or geriatric patient.
• Fluency in English required.

Responsibilities

• Coordinate and implement discharge planning for unit patients.
• Obtain continued-stay authorizations from payer/MCO.
• Facilitate patient transitions through continuum of care.
• Plan and lead family assessment meetings; conduct family/individual therapy when feasible.
• Provide education for staff within social work scope.
