Job summary

Dermacore Wound Care in California is seeking a Nurse Practitioner to join our mobile wound care team on a per diem basis. You will deliver hands-on wound care in patients' homes and community settings, balancing clinical independence with interdisciplinary support.

You will assess wounds, develop and adjust treatment plans, document thoroughly, and coordinate care with the broader team. Travel within the service area and HIPAA compliance are essential; W-2 employment with per-visit compensation

Qualifications

• Current and unrestricted Nurse Practitioner license in the applicable state.
• Current national certification and prescribing authority, as applicable.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Excellent communication and documentation skills.
• Ability to independently manage patient care in a mobile environment.
• Reliable transportation and willingness to travel within service area.
• Wound care experience preferred; additional training may be provided.

Responsibilities

Per Diem Nurse Practitioner - Mobile Wound Care Position

Dermacore Wound Care is seeking a Nurse Practitioner (NP) to join our mobile wound care team on a per diem basis. This role provides hands-on, patient-centered wound care in patients’ homes and other community-based settings, combining clinical independence with the support of an experienced interdisciplinary team.

Our providers take ownership of the full patient experience - from assessment and treatment to documentation, communication, coordination, and follow-up. We are looking for a provider who is confident working independently in the field while staying connected and collaborative with the broader care team.

What You'll Do

• Perform comprehensive patient and wound assessments and develop individualized treatment plans.
• Provide wound care visits based on each patient’s clinical needs and established plan of care.
• Perform wound care procedures within your scope of practice, training, and clinical privileges.
• Develop, update, and manage wound-related treatment recommendations and orders as appropriate.
• Monitor wound progression and adjust treatment plans when clinically indicated.
• Complete accurate and thorough clinical documentation within required timelines, with an expectation of same-day charting.
• Communicate with patients, families/caregivers, referring agencies, physicians, and other members of the care team regarding treatment, progress, and changes in condition.
• Collaborate with internal Clinical Services, Care Coordination, Medical Records, and other support teams to promote continuity of care.
• Recognize changes in patient condition and appropriately elevate clinical concerns or recommend a higher level of care when necessary.
• Participate in required clinical education, training, quality initiatives, and team meetings.
• Maintain compliance with Company clinical standards, HIPAA requirements, and applicable federal and state regulations.

What We're Looking For

• Current and unrestricted Physician Assistant license in the applicable state.
• Current national certification and prescribing authority, as applicable.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and documentation skills.
• Ability to independently manage patient care in a mobile environment while collaborating effectively with an interdisciplinary team.
• Reliable transportation and willingness to travel throughout the assigned service area.
• Ability to maintain all required licensing, credentialing, certifications, and continuing education requirements.
• Wound care experience preferred; additional wound care education and training may be provided based on experience.

Schedule & Travel

This is a per diem position with flexible scheduling based on provider availability, patient census, geographic coverage, and operational needs.

Patient volume and assignments may vary, and a minimum number of patient visits or hours is not guaranteed. Local travel to patient homes and/or facilities within the assigned service area is required. Travel of up to approximately 55 miles may be required depending on patient location and geographic coverage.

Mileage reimbursement is provided for eligible business travel in accordance with Company policy.

No routine on-call requirement.

Compensation

• $100 per completed patient visit
• Required trainings, meetings, and designated non-visit work are compensated separately at $100 per hour

The per-visit rate includes the work associated with completing the patient visit, including the clinical encounter, required documentation and charting, communication with agencies and other members of the care team, communication with patients and families/caregivers as appropriate, and routine follow-up associated with the visit.
