Job summary

The Beach Post-Acute in Long Beach, CA is seeking a Per-Diem Physical Therapist to join our Rehab team. The role involves performing evaluations, developing care plans, and advocating for residents and their families.

Qualifications

• CA PT license or DPT active required.
• Minimum 1 year SNF/LTC experience preferred.
• Excellent customer service and patient care skills.

Responsibilities

• Provide evaluations and treatments, develop care plans.
• Advocate for the rehab team, patients, and families.
• Document daily progress, monthly reports and evaluations.
• Screen individuals for potential therapeutic intervention.
• Provide PTAs with clear directives on treatment plans.
