Job summary

Human Touch Home Health seeks compassionate Physical Therapists (PTs) for PRN home health opportunities across Carbon County, Pennsylvania. You choose the cities to cover, share availability, and we match you with patient opportunities in your preferred territory.

Complete PT evaluations in patients' homes, develop plans of care, and provide therapeutic exercise, gait, balance and mobility training, with mileage reimbursement and flexible scheduling to fit your availability.

Qualifications

• Active Pennsylvania Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical and communication skills.
• Ability to work independently in patients' homes.

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength, endurance, mobility, and independence
• Assess home safety and identify fall risks
• Develop and reinforce home exercise programs
• Educate patients, families, and caregivers
• Monitor patient progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Collaborate with nurses, physicians, therapists, and other members of the care team
• Complete accurate and timely documentation using WellSky

Patient opportunities may be available in Jim Thorpe, Lehighton, Palmerton, Weatherly, Nesquehoning, Summit Hill, Lansford, Albrightsville, and surrounding communities.

And here's the important part: You do NOT have to cover all of Carbon County.

What You'll Do

• Complete physical therapy evaluations in patients' homes

What We're Looking For

• Active Pennsylvania Physical Therapist License Required

• Graduate of an accredited Physical Therapy program

• Valid Driver's License and reliable transportation

• Strong clinical and communication skills

• Ability to work independently in patients' homes

• Home Health Experience Preferred, but Not Required

• WellSky Experience is a Plus
