Job summary

Interim HealthCare Midatlantic is seeking a Home Health Physical Therapist (PT) to deliver personalized therapy in patients' homes across Carbon County. This role offers per-visit compensation ($65-$125) with weekly pay, clinical autonomy to evaluate and set goals, and a supportive interdisciplinary team.

You'll perform home visits, develop plans of care with measurable goals, educate families, supervise assistants, and document progress to support safe discharge. Equal opportunity employer.

Qualifications

• Must have PA PT license.
• Two years PT experience preferred; home health experience preferred.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
• Strong clinical judgment, communication, organization and documentation skills.

Responsibilities

• Evaluate function, mobility, strength, balance, safety and equipment needs in the home.
• Develop plan of care with measurable short- and long-term goals.
• Provide therapeutic exercise, gait and transfer training, and other interventions per the plan of care.
• Reassess at each visit and obtain revised orders when indicated.
• Educate patients, families and caregivers on exercises, fall prevention and safe mobility.
• Supervise PT assistants as assigned.
• Document treatment, response and progress accurately and on time; support discharge planning.

What You'll Do

As a Home Health PT, you will deliver personalized therapy that helps patients move safely and confidently at home:

• Evaluate function, mobility, strength, balance, safety, and equipment needs in the home
• Help develop the plan of care with measurable short- and long-term goals
• Provide therapeutic exercise, gait and transfer training, and other interventions per the plan of care
• Reassess at each visit, complete re-evaluations, and obtain revised orders when indicated
• Educate patients, families, and caregivers on exercises, fall prevention, and safe mobility
• Supervise physical therapist assistants as assigned
• Document treatment, response, and progress accurately and on time, and support discharge planning

What You Bring

• Graduate of a CAPTE-accredited physical therapy program with a current Pennsylvania PT license
• Two years of physical therapy experience relevant to the patient population within the past five years preferred
• Home health experience preferredCurrent CPR certification
• Valid driver's license, auto insurance, and reliable transportation
• Strong clinical judgment, communication, organization, and documentation skills
