Job summary

Advantage Home Health Services in Centre County, PA is hiring a Physical Therapist for full-time work in patients' homes. You will provide PT services, assess and treat functional deficits, and guide patients toward independence while ensuring high-quality rehabilitation.

A licensed Physical Therapist, NBCPT-certified or eligible, will join a team focused on compassionate, patient-centered care with opportunities for growth and ongoing education.

Qualifications

• NBCPT certification required or eligible; active license in PA or practicing state.
• Current Physical Therapy license in the practicing state is required.

Responsibilities

• Provide physical therapy services to patients as prescribed by the physician.
• Examine, test, and treat patients with acute or chronic physcial dysfunction or pain to maximize functional independence.
• Promote patient progression and independence in the least restrictive environment.
• Supervise physical therapist assistants and ensure safe, high-quality rehabilitation services.
• Ensure services align with federal, state, and local standards and the Agency's mission.

Responsibilities

What You'll Do

• To provide physical therapy services to patients as prescribed by the physician.
• To assist in examining, testing, and treating patients who present acute or prolonged physical dysfunction or pain to maximize functional independence.
• To promote and assist patients in progressing care to achieve optimum patient involvement and independent maintenance of their needs in the least restrictive environment.
• To ensure quality and safe delivery of rehabilitation services and provide supervision to physical therapist assistants.
• To ensure that physical therapy services reflecting the Agency's mission and standards are provided in accordance with federal, state, and local standards, guidelines, and regulations to demonstrate support for achieving the mission of the Agency.

Qualifications

About You

• You have completed a program approved by the National Board for Certification in Physical Therapy
• You have a current Physical Therapy license issued by the state in which you are practicing
