General Summary

Under the supervision of the Clinical Therapies Nursing Supervisor, the Physical Therapist provides medically prescribed physical therapy to patients in their place of residence and offers advisory services to other professionals involved in patient care.

Responsibilities

• Deliver physical therapy services to patients aged adolescent through geriatric, following home health division policies.
• Assess patient data, interpret age‑specific needs, and develop individualized care plans.
• Provide advice and support to multidisciplinary team members.
• Maintain accurate documentation and communication with staff, patients, and families.

Education

Minimum of a Bachelor of Science degree in Physical Therapy from an accredited school.

Experience

Preferred: One year of physical therapy experience.

Special Employment Requirements

• Current licensure as a registered Physical Therapist in Pennsylvania.
• Clear PA State Police criminal history check.
• Flexibility to manage multiple responsibilities and function independently in home settings.
• Good written and verbal communication and interpersonal skills.
• Valid Pennsylvania driver’s license and reliable transportation during all working hours.
• Physical ability to bend, reach, stoop, transport self, and travel to patient homes.
