Job summary

The Gardens at Monroe Healthcare is seeking a dedicated Physical Therapist (PT) to join our team on a per diem basis in Middlesex. This role offers an opportunity to make a meaningful impact on residents’ mobility and quality of life.

As a Physical Therapist, you will evaluate residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to support optimal outcomes. You will also document patient progress and educate families on treatment plans.

Qualifications

• Active Physical Therapist license in the State of New Jersey.
• Prior long-term care experience required.
• Strong clinical, communication, and documentation skills.
• Ability to work independently and as part of a team.
• Flexible availability, including weekdays and weekends.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy to improve mobility, strength, and functional abilities.
• Collaborate with interdisciplinary team members to support optimal outcomes.
• Document patient progress in accordance with facility standards.
• Educate residents and families on treatment plans and exercises.
• Ensure compliance with safety and regulatory requirements.
