Job summary

Day Kimball Health in Putnam, CT seeks a per diem Physical Therapist in our Inpatient department. Under the Physical Medicine Director, you will implement prescribed therapy plans to reduce disability and maximize function, while upholding teamwork and patient-centered care.

Responsibilities include evaluating patients, documenting progress, educating families, coordinating with physicians, and mentoring students and staff; CT licensure and CPR are required, with a minimum 1 year experience

Qualifications

• BS, MS or DPT degree from an accredited Physical Therapy program.
• Licensure by the state of Connecticut.
• Current AHA Healthcare Provider CPR certification or equivalent within 3 months of hire.
• Minimum of one-year clinical experience preferred.

Responsibilities

• Evaluates and treats patients as prescribed by referring physician.
• Records treatment, response, progress and discharge information.
• Educates patient, family and significant others regarding condition, treatment and home program.
• Confers with physician and practitioners regarding progress or changes.
• Instructs and directs assistants and students.
• Participates in department meetings and continued education.
• Assists with planning and implementing new programs.
