Job summary

Day Kimball Health in Putnam, CT is seeking a per diem Physical Therapist to join the Inpatient department. You will organize and administer prescribed physical therapy programs to reduce disability and help patients achieve their maximum functional potential.

You will evaluate and treat patients, document progress, educate patients and families, and coordinate with physicians. Connecticut licensure and CPR within 3 months are required; 1 year of experience preferred.

Qualifications

• Ability to review referral, patient condition and records.
• Ability to create and implement PT treatment plans.
• Strong skills in documentation and progress notes.

Responsibilities

• Evaluates and treats patients as prescribed by physician.
• Records treatment, progress and discharge information.
• Educates patient and family regarding condition and home program.
• Confers with physician and staff about progress and changes.
• Orients and directs assistants and students.
• Maintains professional growth through meetings and inservice.
