Job summary

JAG Physical Therapy is seeking a Per Diem Physical Therapist to support outpatient orthopedic services in New Jersey. You will evaluate patients, develop and implement individualized treatment plans, and document care using Prompt EMR.

The role emphasizes collaboration, professionalism, and a proactive approach to restoring function and reducing pain. The position requires NJ licensure, a degree in physical therapy, and CEU compliance.

Qualifications

• Bachelor's or Master's degree required; Doctorate preferred
• Licensed Physical Therapist in NJ
• CEU requirements completed for NY/NJ
• Valid CPR license
• Experience with EMR software preferred
• Excellent oral and written communication
• Strong clinical decision-making skills
• Positive work ethic and dedication to patient success
• Basic computer skills
• Ability to work independently and as part of a team
• Maintains a professional demeanor at all times

Responsibilities

• Collect and record patient care data per guidelines and reimbursement requirements
• Perform initial evaluation, daily notes, progress reports, and discharge plans
• Record daily billing and follow productivity guidelines
• Prepare for evaluations per department policies and research diagnoses
• Select and administer tests and measures
• Synthesize evaluation results to form diagnoses, goals, prognosis and plan of care
• Establish treatment plans and collaborate with interdisciplinary team
• Identify need for consultations and refer to other health care professionals
• Provide education to patients and families
• Functional training for ADLs/IADLs, transfers, rehab equipment use
• Prescribe and progress exercises for patient caseload
• Provide safe, effective interventions in line with JAG policies
• Perform musculoskeletal interventions and modalities
• Document treatments using Prompt EMR
• Attend JAG Billing and Coding seminars
• Maintain respectful interactions with patients and team
