Job summary

JAG Physical Therapy is seeking a Per Diem Physical Therapist to support staffing across our Pennsylvania network. You will assess patients, design treatment plans, and document progress using Prompt EMR while collaborating with a multidisciplinary team to maximize functional outcomes.

The ideal candidate holds PA licensure, a Bachelors or Masters in physical therapy (Doctorate preferred), CPR certification, and EMR experience.

Qualifications

• Bachelors or Masters degree in Physical Therapy; Doctorate preferred.
• Licensed Physical Therapist in the state of practice.
• CPR certification and ability to complete CEUs as required.
• Experience with EMR software and strong communication skills.

Responsibilities

• Perform initial evaluations and ongoing assessments of patients.
• Develop and implement treatment plans to improve function and reduce pain.
• Document care plans, progress notes, and discharge plans per guidelines.
• Collaborate with multidisciplinary team to optimize patient outcomes.
• Provide patient education and maintain high standards of care.
• Ensure compliance with billing, coding, and department policies.
