Job summary

Center of Illinois Home Healthcare, Inc. is seeking a Per Diem Physical Therapist Assistant (PTA) to provide skilled services in patients’ homes in Arlington Heights, IL. This role requires an IL PTA license and CPR; experience in home health highly preferred.

You will work under a licensed Physical Therapist, following the plan of care, documenting patient progress, and collaborating with a multidisciplinary team to ensure quality care and safety in the home setting.

Qualifications

• Current Illinois PTA license required.
• Minimum of one year of home health experience preferred.
• Knowledge of Medicare home health regulations and documentation preferred.
• Experience with Axxess EMR is a plus.
• Strong clinical, communication, and organizational skills.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Follow the established plan of care and treatment goals developed by the supervising Physical Therapist.
• Observe, document, and report patient responses to treatment and communicate changes in condition to the supervising Physical Therapist.
• Educate patients and caregivers on prescribed exercises, mobility techniques, fall prevention, and home safety.
• Complete accurate and timely clinical documentation in accordance with Medicare, CMS, and agency guidelines.
• Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary care team.
• Participate in care coordination and case conferences as needed.
• Maintain compliance with agency policies, state regulations, and accreditation standards.
