BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a Per Diem opportunity throughout Baltimore County, Maryland.

This per diem opportunity requires current home health and Oasis experience.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care at home.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic areas.
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns and appearance, muscle strength and endurance, balance testing, and safety assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the care plan, collaborating with nursing in the multidisciplinary care team as appropriate.
• Educate clients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.

Qualifications for a PT - Physical Therapist include:

• Current Maryland Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-approved program or the Committee on Allied Health Education and Accreditation of the American Medical Association, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
