VNA & Hospice of the Southwest Region is seeking a per diem Pediatric Physical Therapist to serve clients in Franklin County, Vermont, delivering home health PT services under the Kids on the Move Clinical Manager. Responsibilities include evaluations, plan of care development, and supervision of students and rehab staff.

The role emphasizes clinic and home-based visits, coordination with a broader care team, and school-based service delivery with associated paperwork and documentation.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Vermont State Physical Therapist license required.
• Valid State Driver's license; background checks required.

Responsabilités

• Actively manages patient visit schedule to maximize productivity and patient need.
• Performs comprehensive PT assessment including psychosocial, educational, safety and risk factor parameters.
• Develops a plan of care to promote functional independence and safety.
• Provides skilled physical therapy treatment and patient/caregiver education; monitors progress.
• Coordinates patient care with other clinical team members and rehab services.
• Handles school-based services: evaluations, IEPs, meetings, and insurance paperwork.
• Coordinates safe patient discharges and transfers.
• Serve as a Home Care PT as required.
