Job summary

The VNA & Hospice of the Southwest Region in Vermont is seeking a per diem Pediatric Physical Therapist to evaluate, plan, and administer physical therapy services in clients' homes or schools within Bennington County. You will work under the Kids on the Move Clinical Manager and may supervise students and rehab aides.

Qualifications

  • Bachelor's degree in Physical Therapy from an accredited university or college required.
  • Vermont State Physical Therapist license required.
  • Valid State Driver's license required.
  • Ability to pass pre-employment background checks.

Responsibilities

  • Actively monitors and adjusts patient visit schedules to maximize productivity and meet patient needs.
  • Performs comprehensive PT assessments including psychosocial, educational, safety, and risk parameters.
  • Develops and implements plans of care to promote optimal functional independence and safety.
  • Provides skilled PT treatment and patient/caregiver education during visits.
  • Coordinates care with physicians, nurses, and other rehab staff to ensure quality patient outcomes.
  • Completes school-based services, including evaluations, IEPs, and related paperwork.
  • Coordinates safe discharges/transfers and serves as Home Care PT as required.

Education

Bachelor's degree in Physical Therapy
Master's degree (preferred)

A Day in the Life of a Pediatric Physical Therapist

Under the direction of the Kids on the Move Clinical Manager, you evaluate, plan, and administer medically prescribed or educationally relevant physical therapy treatment to clients in their homes or students in their respective educational environment. In addition, this position is responsible for clinical supervision of students, physical therapy assistants and rehabilitation aides. Responsibilities include but are not limited to:

Key Responsibilities
  • Actively monitors and amends patient visit schedule to maximize productivity and patient need. Acts to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc.
  • Performs a comprehensive PT assessment of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
  • Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
  • Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.
  • As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record.
  • Accurately and completely performs functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork in a timely manner.
  • Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed.
  • Serve as a Home Care PT as required.
Qualifications
  • Bachelor's degree in Physical Therapy from an accredited university or college required. Master's degree preferred.
  • Vermont State Physical Therapist license required.
  • Valid State Driver's license required. Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.
