Job summary

AdvisaCare in Michigan seeks a compassionate Social Worker to guide patients and families through hospice care with advocacy and thoughtful support. You will provide psychosocial assessments, counseling, and crisis intervention while collaborating with the care team to ensure patient-centered plans.

The role requires an MSW with LMSW/LLMSW licensure in Michigan, experience with home health or hospice, and reliable transportation.

Qualifications

• MSW degree from a CSWE-accredited program.
• Active LMSW or LLMSW license in Michigan.
• Prior experience with patients and families in a care setting.
• Valid driver's license, car insurance, and reliable transportation.

Responsibilities

• Perform psychosocial, emotional, spiritual, and bereavement assessments and assist in goal-directed care plans.
• Provide care under physician direction and participate in IDT conferences.
• Document visits and referrals in the patient record accurately and timely.
• Conduct reassessments and provide counseling as needed.
• Offer crisis intervention and individual or family counseling when indicated.
• Advocate for patients and families and coordinate with community services for referrals.

In this role, you will help guide patients and families through the hospice journey with compassion, advocacy, and thoughtful support. Responsibilities

• Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist in the development and implementation of goal-directed care plans.
• Provide care under the direction of a physician and participate in IDT conferences, observing, assessing, and sharing information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family.
• Document all patient and family visits, telephone contacts, and referral actions in a timely, accurate, and concise manner in the patient record.
• Conduct reassessments of patient and family needs and provide counseling as required.
• Provide crisis intervention and individual or family counseling when indicated.
• Serve as a care advocate for the patient and family unit.
• Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate related referrals.
• Provide ongoing counseling related to issues of death and dying to the patient and family as needed.
• Participate as a member of the Bereavement Team as assigned.
• Adhere to all hospice policies.

Requirements

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• Active LMSW or LLMSW license in Michigan
• Prior experience working with patients and families
• Valid driver's license, car insurance, and reliable transportation
• Eligible to work in the United States
• Experience in home health care or hospice care preferred
