Job summary

AdvisaCare is seeking a qualified Hospice Social Worker to guide patients and families through the hospice journey with compassion and advocacy. The role emphasizes psychosocial assessment, counseling, and community referrals in a home health care setting.

You will work under physician supervision, participate in IDT conferences, document visits, and provide crisis support as needed. A CSWE-accredited MSW and Michigan LMSW/LLMSW license are required.

Qualifications

• MSW degree from a CSWE-accredited program.
• Active LMSW or LLMSW license in Michigan required.
• Experience working with patients and families in hospice or home health settings preferred.
• Valid driver''s license, car insurance, and reliable transportation.

Responsibilities

• Perform psychosocial, emotional, spiritual, and bereavement assessments and contribute to goal-directed care plans.
• Collaborate with physicians and participate in IDT conferences, sharing psychosocial and financial updates.
• Document all patient and family visits and communications in the patient record.
• Conduct regular reassessments and provide counseling as needed.
• Provide crisis intervention and individual or family counseling when indicated.
• Advocate for patient and family care needs and coordinate with community resources.
• Engage with the Bereavement Team as assigned and support related activities.
• Adhere to hospice policies and maintain high-quality care standards.

In this role, you will help guide patients and families through the hospice journey with compassion, advocacy, and thoughtful support. Responsibilities

• Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist in the development and implementation of goal-directed care plans


• Provide care under the direction of a physician and participate in IDT conferences, observing, assessing, and sharing information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family


• Document all patient and family visits, telephone contacts, and referral actions in a timely, accurate, and concise manner in the patient record


• Conduct reassessments of patient and family needs and provide counseling as required


• Provide crisis intervention and individual or family counseling when indicated


• Serve as a care advocate for the patient and family unit


• Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate related referrals


• Provide ongoing counseling related to issues of death and dying to the patient and family as needed


• Participate as a member of the Bereavement Team as assigned


• Adhere to all hospice policies

Requirements

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)


• Active LMSW or LLMSW license in Michigan


• Prior experience working with patients and families


• Valid driver's license, car insurance, and reliable transportation


• Eligible to work in the United States


• Experience in home health care or hospice care preferred
