Job summary

AdvisaCare Home Health and Hospice in Mount Pleasant, MI seeks a Full-time MSW to join our compassionate team. You will assess psychosocial needs, support patients and families, and develop care plans under physician direction.

Ideal candidates hold an CSWE-accredited MSW, have hospice or home health experience, and possess a valid driver’s license. We offer a supportive environment with 401K, medical benefits, PTO and flexible scheduling.

Qualifications

• MSW degree from a CSWE-accredited program.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid Driver's License, car insurance and reliable transportation.

Responsibilities

• Performs psychosocial, emotional, spiritual and bereavement assessments and helps develop goal-directed care plans.
• Assists under physician direction; participates in IDT conferences and shares psychosocial, spiritual, emotional, physical and financial patient/family information.
• Documents patient/family visits, calls and referrals in the patient record.
• Conducts reassessments and counseling for patient/family needs.
• Provides crisis intervention and individual or family counseling when indicated.
• Acts as a care advocate for patient/family unit.
• Develops/maintains contact with community agencies to facilitate referrals.
• Provides counseling on death and dying as needed.
• Participates in Bereavement Team as assigned.
• Adheres to Hospice policies.

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• One year of experience in Home Health Care preferred

Job Duties:

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans
• Provides care under the direction of a physician. Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
• Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record.
• Conducts reassessments of patient/family needs and counseling as required.
• Provides crisis intervention and individual or family counseling when indicated.
• Plays an active role as a care advocate for the patient/family unit.
• Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
• Provides ongoing counseling related to issues of death and dying to the patient and family as needed.
• Participates as a member of the Bereavement Team as assigned.
• Adheres to all Hospice policies.
