Job summary

Apple Valley Health & Rehab in Yakima, WA is seeking a compassionate Occupational Therapist (OT) to join our team on a per diem basis. You will deliver skilled OT services to residents to improve independence and functional abilities in a rehabilitation setting.

The ideal candidate collaborates with an interdisciplinary team, maintains thorough documentation, and adheres to regulatory standards to ensure high-quality patient care and safety.

Qualifications

• Active OT license in WA or eligibility for WA licensure.
• Graduate of an accredited OT program.
• CPR/BLS certification required or obtainable.
• Experience in skilled nursing or rehab setting preferred.

Responsibilities

• Evaluate residents to identify functional limitations and goals.
• Develop and implement individualized OT treatment plans.
• Provide interventions to improve ADLs, strength, mobility and independence.
• Monitor progress and adjust plans as needed.
• Document evaluations, sessions, progress and discharge information.
• Collaborate with nursing, PT, SLP, physicians, and families.
• Educate residents and families on techniques and home programs.
• Maintain compliance with federal/state regulations and facility policies.
• Promote safety, dignity, independence, and quality of life.
