Job summary

Constellation Home Care is seeking a Per Diem Occupational Therapist (OT) to work with our home care team in Norwak, CT! This field-based position aims to develop, recover or maintain daily living and work skills for patients with physical, mental or developmental conditions in their homes.

Responsibilities include assessing and planning care, educating patients and caregivers, documenting in the EMR, and contributing to an individualized plan of care.

Qualifications

• Licensed OT in the state of employment.
• 1–2 years of clinical experience; home care experience preferred.
• Oasis experience strongly desired.
• Have reliable transportation and willingness to commute for in-home care.

Responsibilities

• Provide Occupational Therapy (OT) to patients by assessing and understanding the care plan.
• Evaluate patient’s functional status and home environment for hazards or barriers to independent living; identify equipment needs.
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
• Participate in development of the individualized plan of care.
• Maintain and process all documentation in compliance with agency standards using the EMR system.
• Submit all clinical documentation, time records and other information in the required time frame.
• Support the company mission by delivering high-quality home care.
