Job summary

OhioHealth is seeking an Occupational Therapist for home health to provide skilled, one-on-one patient care in the home setting. You will collaborate with an interdisciplinary team, maintain a flexible schedule, and participate in a limited weekend rotation with on-call expectations.

Responsibilities include evaluating and treating patients, educating families, coordinating discharge plans, and contributing to department quality improvement. Some travel to other care sites may be required.

Qualifications

• Bachelor's Degree is required.
• BLS certification is required.
• LIC – licensed in Ohio.
• OT – American Occupational Therapy Association certification.
• Driver's license if in Home Care department and minimum 18 years old with MVR proof.

Responsibilities

• Direct patient care (approximately 90%) with evaluation and treatment of motor, cognitive, sensory, and psychosocial function.
• Document evaluation, plan of care, progress, and discharge plans for the multidisciplinary team.
• Supervise COTA staff and rehab technicians; participate in quality improvement and staff orientation.
• May work at care sites other than the home care site as needed.

Home health OTs provide skilled care to patients to achieve outcomes in collaboration with the patient’s provider. There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.

Responsibilities And Duties

• Driving is an essential function of the job. 90% Direct Patient Care - Evaluates and treats patients after physician referral to: Evaluate patient’s motor, cognitive, sensory integrative and/or psychosocial function. Develops and implements plan of care based on evaluation findings and appropriate treatment techniques. Implements plan of care based on appropriate occupational performance frame of reference(s): bio-mechanical, sensorimotor, and/or rehabilitative. Attends multidisciplinary patient care meetings. Plans and coordinates discharge plan with multidisciplinary team. Educates patient and caregiver/family. Recommends Medical Equipment required for discharge. Provides information to physicians, nurses, case managers, social workers, and other team members.
• 5% Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties.
• 5% - Indirect Department Functions: Supervises patient assignments and duties of Certified Occupational Therapy Assistants (COTA) and rehab technicians. Is involved in department quality improvement through utilization review and quality assurance program. Takes active role in orienting new staff members. Serves as clinical instructor for OT and COTA students. Educates other disciplines and medical/hospital personnel on the topic of OT purpose, goals and treatment policies and procedures. Represents department at problem-solving and committee meetings. Assumes other duties delegated by management.
• There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.

Minimum Qualifications

• Bachelor's Degree (Required)
• BLS - Basic Life Support - American Heart Association
• DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles
• LIC - Licensed in Field - State of Ohio
• OT - Occupational Therapist - American Occupational Therapy Assoc.

Specialized Knowledge

• Knowledge of home health Medicare regulations, JCAHO standards, home care reimbursement
• Experience in an acute, rehabilitation or home health setting
• Basic personal computer skills
• Knowledge in assessment, planning, implementation, and evaluation of occupational therapy
• BLS certification
• For this position, in the Home Care department only, Minimum Qualifications include valid state driver’s license, minimum of eighteen (18) years old, have an acceptable Motor Vehicle Report (MVR), and provide proof of insurance, if applicable.
