Job summary

The Christ Hospital Home Health Care is seeking an Occupational Therapist for a part-time role (1 day per week) in the Cincinnati area. You will deliver home-based therapy, educate patients and families, and ensure safe, compliant care under physician orders.

Includes a supportive interdisciplinary team, flexible scheduling, and opportunities with a broad benefits package. OT licensure in Ohio and at least 1 year in acute care are required; driver’s license is essential.

Qualifications

  • Occupational Therapist with a current license in the state of employment.
  • Minimum one-year experience as an OT in an acute care setting.
  • Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.

Responsibilities

  • Plan of Care: Provide skilled occupational therapy as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
  • Develop an individualized OT plan of care and make adjustments as needs change.
  • Evaluation: Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
  • Patient Outcomes: Use activities and home exercise programs to improve ROM, strength, and coordination.
  • Minimize Patient Risk: Help decrease re-hospitalizations by teaching preventative measures.
  • Family Educator/Advocate: Counsel patient and family on disease/injury and management.
  • Instruct patient and family on proper use of mobility devices.
  • Supervision: Oversee and supervise total care provided by COTA.
  • Communication: Collaborate with COTA on supervisory visit results.
  • Policies/Regulations: Complete documentation per agency and Medicare guidelines.
  • Operations: Attend in-service training and mandatory meetings.

OCCUPATIONAL THERAPIST (OT) SCHEDULE: Part-Time, 1 Day per Week, Monday-Friday, 8:15 AM -5:00 PM. No On Call, Weekend, or Holiday Requirements. TERRITORY: Cities within Brown, Clermont, Clinton, Hamilton, Highland, and Warren Counties. AGENCY: The Christ Hospital Home Health Care.

HOW YOU'LL WORK

You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.

MAJOR AREAS OF RESPONSIBILITY
  • Plan of Care: Provide skilled occupational therapy as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
  • Develop an individualized OT plan of care and make adjustments as needs change.
  • Evaluation: Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
  • Patient Outcomes: Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.
  • Minimize Patient Risk: Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
  • Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy.
  • Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Supervision: Oversee and supervise total care of patient provided by COTA.
  • Communication/Collaboration: Communicate with the COTA on supervisory visit results.
  • Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Operations: Attend in-service training and mandatory agency meetings.

  • Compassionate communicator with a positive attitude.
  • Patience is a virtue when working with patients, families, physicians, and coworkers.
  • Attention to detail is critical, as is being observant and following directions.
REQUIREMENTS
