Job summary

Mercy Medical Center Home Health and Hospice is seeking an Occupational Therapist to provide skilled home-based therapy under physician direction. You will educate patients and families, ensure safety, and document care in compliance with Medicare guidelines. A valid OT license in Ohio and a minimum of one year in acute care/home health settings are preferred.

Responsibilities

  • Plan of Care: Develop and adjust the OT plan of care as directed by the attending physician.
  • Evaluation: Assess patient functioning and progress using appropriate measures.
  • Patient Outcomes: Improve range of motion, strength, and coordination through therapeutic activities.
  • Minimize Patient Risk: Teach preventative measures to reduce hospitalizations.
  • Family Educator/Advocate: Educate patient and family on disease/injury and home care.
  • Supervision: Oversee COTA care and share supervisory visit results.
  • Policies: Complete documentation per agency protocol and Medicare guidelines.
  • Rules and Regulations: Follow agency policies and communicate schedule changes.
  • Operations: Attend in-service trainings and mandatory meetings.

How You'll Make a Difference

You'll provide home-based therapy care for patients as directed by the attending physician. How You'll Work

You’ll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.

Major Areas Of Responsibility
  • Plan of Care: Provide skilled occupational therapy as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Develop an individualized OT plan of care and make adjustments as needs change.
  • Evaluation: Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
  • Patient Outcomes: Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.
  • Minimize Patient Risk: Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
  • Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy. Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Supervision: Oversee and supervise total care of patient provided by COTA.
  • Communication/Collaboration: Communicate with the COTA on supervisory visit results.
  • Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Operations: Attend in-service training and mandatory agency meetings.

Requirements
  • Occupational Therapist with a current license in the state of employment.
  • Minimum one-year experience as a OT in an acute care setting. Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands
