Job summary

OhioHealth is seeking a Physical Therapist for inpatient rehabilitation. The role involves evaluating patients, developing and implementing plans of care, and coordinating discharge planning with the multidisciplinary team.

Job Description Summary:

To competently interpret the patients history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. Based on problems identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Actively participates in departmental functions.

Responsibilities And Duties:
  • Direct Patient Care 90 - 100% Evaluates and treats patients: After provider referral, evaluates the patient's physical abilities and functional status, develops and implements plan of care based on evaluation findings and appropriate treatment technique and initiates patient treatment. Carries out plan of care with reassessment and changes to plan of care as appropriate. Therapist is responsible for maintaining/meeting the productivity expectation of their department. Plans and helps coordinate discharge plan with multidisciplinary team: Instructs patient and care giver/family. Recommends DME required for discharge. Provides information to physicians, nurses, case managers, social workers and other team members. Involved in the multidisciplinary patient plan of care and discharge readiness plan. Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance policies.

  • Indirect Patient Care 1-10% Department Functions: Supervises patient assignments and duties of the PTAs and rehab techs. Is involved in department quality improvement through data collection and participation in team and staff meetings. Assumes duties delegated by management in the absence of department management, as needed. There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.

Education/Mentoring:

Takes active roles in orienting new staff members. Serves as clinical instructor for PT and PTA students. Serves as an advocate for discipline on the multidisciplinary team. Represents department at problem-solving and committee meetings. Minimum Qualifications:
  • Bachelor's Degree: Physical Therapy (Required)
  • DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles, LIC - Licensed in Field - State of Ohio, PT1 - Physical Therapist License - American Board of Physical Therapy Specialties
  • Additional Job Description: Licensed in the State of Ohio. Basic computer skills. Entry level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical Experience.
