Job summary

Optum Home & Community Care in the United States is seeking a Nurse Practitioner or Physician Assistant to deliver personalized care in skilled nursing, assisted living, and senior housing communities. You will build relationships with patients and collaborate with an interdisciplinary care team to improve outcomes for older adults.

The role offers a $5,000 sign-on bonus for external candidates, competitive pay from $60–$84/hour plus bonus opportunities, and a comprehensive benefits package

Qualifications

• Active ANCC or AANP certification as a Nurse Practitioner
• Active NCCPA certification for Physician Assistants
• Active state license (or able to obtain before start)
• DEA license or ability to obtain
• Valid driver’s license and reliable transportation
• Experience in geriatrics, long-term care, home care, or senior living preferred
