Responsibilities

  • Perform comprehensive patient assessments including medical history, exams, and vital signs.
  • Coordinate care plans with interdisciplinary teams.
  • Lead advance care planning discussions and educate patients and families.
  • Support short-term sub-acute rehabilitation stays.
  • Maintain accurate clinical documentation including ICD-10 coding.

$5,000 Sign-On Bonus for External Candidates

Seeking clinicians available for at least 24 hours per month. Of those 24 hours, 16 hours must be dedicated to off-shift coverage.

The Senior Community Care (SCC) program provides coordinated, comprehensive care to some of our highest-risk health plan members. Key Responsibilities
  • Complete comprehensive patient assessments, including:
    • Review of medical history and medications
    • Physical examinations and vital signs
    • Appropriate screenings and point-of-care testing
  • Support members through short-term sub-acute rehabilitation stays
  • Maintain thorough and accurate clinical documentation, including ICD-10 coding
  • Partner closely with interdisciplinary care teams to coordinate treatment plans
  • Lead discussions around advance care planning and serious illness care
  • Educate and counsel patients, families, and caregivers

Required Qualifications

For Nurse Practitioners:

  • Current ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

For Physician Assistants:

  • Active NCCPA certification and applicable state licensure

Additional requirements:

  • Unrestricted state license (or ability to obtain before start date)
  • Valid driver's license with reliable transportation
  • Active DEA license/prescriptive authority, or ability to obtain after hire in accordance with state requirements
  • Ability to establish a collaborative practice agreement when required
  • Ability to work in a variety of clinical settings and perform essential job functions
Preferred Qualifications
  • One or more years of APC experience, preferably in long-term care
  • Experience in geriatric care, senior living, home care, or long-term care environments
  • Familiarity caring for patients with complex medical, behavioral, social, or functional needs
  • Knowledge of chronic disease management and geriatric medicine
  • Experience facilitating advanced illness and end-of-life conversations
  • Comfort using electronic medical records and healthcare technology

Compensation: Hourly compensation for this position typically ranges from $53 to $79 per hour, based on factors such as location, experience, education, and other job-related considerations. Eligible employees may also receive bonus opportunities and access to a comprehensive benefits package, including 401(k), employee stock purchase programs, recognition programs, and other benefits, subject to eligibility requirements.
