Job summary

Optum Home & Community Care seeks Nurse Practitioners and Physician Assistants to provide in-home care for patients with complex health needs, helping them stay healthier and independent. You will work in patients' homes within a collaborative, interdisciplinary team.

Qualifications

• ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• NCCPA certification and active state licensure
• Active, unrestricted state license (or ability to obtain before start)
• Valid driver’s license and reliable transportation
• Willingness to travel within the local service area
• Passion for delivering patient-centered, relationship-based care

Responsibilities

• Deliver comprehensive in-home assessments and ongoing clinical care
• Manage acute and chronic conditions for medically complex patients
• Partner with physicians, nurses, care coordinators, and social workers
• Educate patients and caregivers on treatment plans and disease management
• Lead meaningful conversations around goals of care and advanced illness planning
