Job summary

Optum Home & Community Care is seeking Nurse Practitioners or Physician Assistants to provide comprehensive in-home care for patients with complex health needs. This field-based role emphasizes autonomy, collaboration with a multidisciplinary team, and strong relationships with patients and families.

You will perform comprehensive assessments, manage chronic conditions, and work with physicians and care coordinators to improve quality of life and reduce hospital visits.

Qualifications

• Active, unrestricted state license (or ability to obtain before start).
• ANCC or AANP or NCCPA certification as applicable to the role.
• Willingness to travel within the local service area.

Responsibilities

• Deliver in-home assessments and ongoing clinical care for medically complex patients.
• Manage acute and chronic conditions in home settings.
• Partner with physicians, nurses, care coordinators, and social workers to coordinate care.
• Educate patients and caregivers on treatment plans and disease management.
• Lead goals-of-care discussions and advance care planning.
• Build long-term relationships with patients and families to support independence.

Education

ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care)

NCCPA certification and active state licensure
