Job summary

Circadia Health is seeking an on-site Nurse Practitioner in Marlborough, MA. You will provide direct patient care, conduct assessments, and coordinate with facility teams to translate predictive insights into timely actions.

You will train staff on Circadia integration, interpret trend data, and ensure timely documentation, aiming to reduce readmissions and improve patient outcomes. The role requires MA licensure and strong independent practice skills.

Qualifications

• Certified and licensed Nurse Practitioner in MA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MA driver’s license; comfortable traveling locally.

Responsibilities

As Circadia’s on‑site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech‑to‑bedside integration through hands‑on training, live demos, and shadowed assessments.

Responsibilities

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day.
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting.
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30–40‑mile radius; for sites outside of that radius, mileage and lodging covered per policy.

Performance Metrics

• Visits: Target 45 patient visits/day.
• Engagement: ≥80% Time to Acknowledgement Rate <24h.
• Documentation: Complete all notes using AI‑powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Qualifications

• Certified and licensed Nurse Practitioner in MA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.
