Job summary

Circadia Health is seeking an on-site Nurse Practitioner in New Jersey to lead direct patient assessments, initiate and adjust care plans, and coordinate with facility teams. You will integrate Circadia’s AI-enabled insights into daily rounds and clinical workflows, driving timely interventions to prevent readmissions.

The role requires NJ NP certification, 2+ years of experience with complex adults, and willingness to travel regionally within the service area.

Qualifications

• Certified and licensed Nurse Practitioner in NJ.
• ANCC certification in Adult/Family/Geriatric/Acute.
• 2+ years NP experience with complex adult populations.
• Valid MI driver’s license; able to travel locally.

Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands‑on training, live demos, and shadowed assessments.

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech to bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30–40‑mile radius; for sites outside of that radius, mileage and lodging covered per policy.

• Visits: Target 25 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

• Certified and licensed Nurse Practitioner in NJ; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MI driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.
