Job summary

Circadia Health is seeking an on-site Nurse Practitioner in New Jersey to see patients, respond to AI-prompted signals, and coach facility teams on data-driven interventions.

You will blend direct patient care with tech-to-bedside integration, including hands-on training and live demos, across SNFs.

Qualifications

• Certified and licensed NP in NJ (ANCC or equivalent).
• 2+ years NP experience with complex adult populations.
• Eligible for Medicare participation and able to travel locally.

Responsibilities

• Conduct initial and follow-up assessments of flagged residents and adjust plans of care.
• Train and coach staff to integrate Circadia into clinical workflows.
• Interpret trend insights and prioritize high-risk patients for timely interventions.
• Complete documentation the same day and ensure accurate charting.
• Perform high-volume visits across SNFs within a 30–40 mile radius.

The Role

Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.

What You’ll Do

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.


• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.


• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.


• Close the loop: Complete documentation/charts within the same day


• High-volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.


• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting


• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.


• Regional flexibility: Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.

How Success is Measured (KPIs)

• Visits: Target 25 patient visits/day


• Engagement: ≥80% Time to Acknowledgement Rate: <24h


• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.


• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Must-Have Qualifications

• Certified and licensed Nurse Practitioner in NJ; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.


• Eligible for Medicare participation; company bills on your behalf.


• 2+ years NP experience with complex adult populations; able to practice independently.


• Valid MI driver’s license; comfortable traveling locally.


• Thrives in a dynamic, evolving startup environment.


• English required

• Base Salary: $120K-140K
