Job summary

Circadia Health is seeking an on-site Nurse Practitioner to lead direct patient care and collaborate with care teams. You will drive real-time health insights into bedside interventions while training staff to use Circadia’s platform within existing workflows.

Ideal candidates are ME-licensed NPs with 2+ years of adult/geriatric care and comfort traveling between facilities. This on-site role requires English proficiency and a dynamic, startup-friendly mindset.

Qualifications

• Licensed Nurse Practitioner in ME with ability to practice independently.
• ANCC or equivalent certification in Adult/Family/Geriatric/Acute care.

Responsibilities

• Direct patient care assessments and plans of care for residents.
• Train and coach facility staff to integrate Circadia into clinical workflows.
• Interpret trend-based insights and prioritize high-risk patients for timely interventions.
• Complete documentation the same day.
• Conduct high-volume rounds (25+ visits/day) focusing on new admits and alerts.
• Provide feedback to improve workflows and tech integration in clinical settings.
• Mentor NP onboarding and collaborate with clinical ops/product teams.
• Cover sites within a 30–40‑mile radius with mileage/lodging covered per policy.

Responsibilities

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day.
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting.
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30–40‑mile radius; for sites outside of that radius, mileage and lodging covered per policy.

Performance Metrics

• Visits: Target 45 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI‑powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Qualifications

• Certified and licensed Nurse Practitioner in ME; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid ME driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.
