Job summary

Bebashi - Transition to Hope in Philadelphia seeks a Nurse Practitioner for its Wellness Center on a per diem basis. The NP will independently assess acute and non-acute problems, perform histories and exams, and develop and implement patient management plans in collaboration with clinical staff.

The role requires RN licensure, PA NP certification, DEA (if prescribing), and BLS/ACLS, with 2–3 years of clinical experience; HIV healthcare experience is preferred.

Qualifications

• Current RN licensure and PA NP Certification (CRNP) required.
• Master's or Doctorate in Nursing from an accredited program.
• Current DEA registration (if prescribing) and BLS/ACLS certification.
• Minimum 2–3 years of clinical experience; HIV healthcare preferred.

Responsibilities

• Independently assesses acute and non-acute clinical problems.
• Develops patient management plans and coordinates care with staff.
• Provides counseling and education to patients and families.
• Orders and interprets labs and diagnostics; documents care plans.
• Collaborates with interdisciplinary team and adheres to HIPAA.
