Job summary

Impact Health PC in Bucks County, PA is seeking a compassionate Palliative Care Nurse Practitioner to join our growing team on a Per Diem basis. You will provide patient-centered care in skilled nursing facilities with physician-level consult access.

The role offers a flexible Monday–Friday schedule, no nights or on-call, competitive salary with bonuses, and robust benefits including 401(k) and malpractice coverage. Active NP license and SNF experience preferred.

Qualifications

• Active NP license in good standing.
• Minimum 2 years post-certification experience.
• Experience in SNFs, hospitalist roles, ED, or ICU preferred.
• Proficient with EHRs and clinical technology.
• Strong communicator with a collaborative mindset.

Responsibilities

• Deliver person-centered care using evidence-based practices.
• Collaborate with interdisciplinary facility teams.
• Conduct assessments, develop treatment plans, and manage follow-up care.
• Provide ongoing follow-up and chronic care management in SNFs.
• Educate patients, families, and staff.
• Complete timely documentation (within 24 hours).
• Compliance with facility, state and federal regulations/protocols.
