Position Description:
Circadia Health is revolutionizing remote patient monitoring for post-acute care and skilled nursing facilities. We are seeking a dedicated and experienced Nurse Practitioner to join our dynamic team. This role will be pivotal in assessing patients health status and providing care plans while utilizing cutting-edge technology to monitor and improve the health outcomes of our long-term care patients.
As a Nurse Practitioner with Circadia Health, you will play a crucial role in our mission to enhance patient care through innovative solutions. You will be responsible for performing in-person patient assessments, developing individualized care plans and collaborating with our interdisciplinary team to ensure the highest standards of care. You'll have the opportunity to travel to designated skilled nursing facilities within your region, fostering direct engagement with patient care and on-site teams.
This is a per-diem position with flexible hours, requiring weekday availability only.
Key ResponsibilitiesCompensation:
This is a pay-per-visit role. Compensation will be based on each completed and signed clinical note in our system.
Rate: $25/completed visits
