Job summary

Harbor Health is seeking a Nurse Practitioner for per diem coverage at our Boston area Health Center locations. The role involves outpatient care, chronic and acute illness management, and referrals to specialists.

You will collaborate with physicians to provide comprehensive patient care and ensure timely documentation in the EMR during visits. Responsibilities include patient education, disease prevention, and participation in care team and quality improvement activities.

Qualifications

• Completed accredited Nurse Practitioner program.
• Current Massachusetts NP license, board-eligible or board-certified.
• Current DEA, MCSR and BLS.
• Computer literacy in MS Office and EMR (EPIC preferred).

Responsibilities

• Provides outpatient care including health maintenance, chronic/acute illness management, and referrals.
• Communicates with patients by mail, email, or phone; collaborates with physicians.
• Documents care in EMR with progress notes, labs, and immunizations; maintains problem lists.
