Job summary

Harbor Health Services, Inc. is seeking per diem Staff Nurses (RNs) to provide as-needed coverage across our Dorchester, Plymouth, and Hyannis health centers in Massachusetts. Shifts include days, some evenings until 8:00 PM, and Saturdays from 8:00 AM to Noon.

The Staff Nurse will perform nursing duties at the top of the license, including triage, care coordination, education, vaccinations, and documentation in the EHR, while adhering to state laws and Harbor Health policies.

Qualifications

• RN license in Massachusetts (preferred BSN)
• Current BLS certification
• Experience in community health setting preferred
• Ambulatory care or health insurance environment experience preferred
• Proficiency with EMR such as Epic/OCHIN EMR
• Excellent customer service and communication skills

Responsibilities

• Performs nursing duties in accordance with Massachusetts Nursing Practice Act and Harbor Health policies.
• Conducts comprehensive assessment and triage in person or by telephone.
• Provides patient education on disease prevention, medications and treatment plans.
• Acts as liaison between patient, care team and provider.
• Provides health coaching for common conditions (e.g., asthma, hypertension, diabetes).
• Documents care activities accurately in the EHR and maintains patient confidentiality.
