Job summary

Bristol Hospice Llc is seeking a compassionate Nurse Practitioner to deliver high-quality hospice care. Join an interdisciplinary team to support patients and families with dignity and comfort in their most important journey.

Per diem schedule with face-to-face visits as needed across Tremonton to North Salt Lake; the role emphasizes patient-centered care, collaboration, and adherence to state Nurse Practice Act.

Qualifications

• Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current nursing licensure in state.
• Master's degree with NP experience and hospice/palliative experience preferred.

Responsibilities

• Provide medical care, palliate and manage terminal illness under supervision of a collaborating physician for adults and children.
• Complete history and physical examinations and order/interpret tests; document findings.
• Develop individualized plan of care, prescribe medications, obtain consultations, and coordinate hospice services.
• Complete hospice face-to-face visit requirements to assess eligibility; report to hospice medical director.
• Review clinical information and coordinate patient care with the interdisciplinary team; ensure symptom control.
• Consult collaborating physician when the plan is outside standardized practice; develop and revise the patient plan of care.
• Fulfill assignments and demonstrate end-of-life communication with patient/family.
