Job summary

Bristol Hospice in Atlanta, GA seeks a compassionate Nurse Practitioner to join our hospice team on a PRN hourly basis. You will provide medical care, palliate and manage terminal illness, and work under a collaborating physician to adults and children.

The role emphasizes patient-centered care, interdisciplinary collaboration, and adherence to state NP practice. You must have a master's in nursing, current NP licensure, hospice experience, and relevant certification.

Qualifications

  • Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
  • Must have current nursing licensure as an NP in the state.
  • Must have a master\'s degree with at least 1 year NP experience and 1 year hospice/palliative care experience.
  • Must hold accreditation from an approved certifying body for advanced practice nursing as required by the state.
  • Certification in a specialty area (e.g., CHPN, Pediatrics, Geriatrics) is preferred.
  • Must be a licensed driver with a registered, insured vehicle.

Responsibilities

  • Provide medical care, palliate and manage terminal illness under supervision of a collaborating physician for adults and children.
  • Perform history and physical exams, order/interpret tests, diagnose conditions and document findings.
  • Develop individualized care plans and coordinate hospice services across disciplines.
  • Complete hospice face-to-face visits as required to assess eligibility.
  • Provide clinical information to hospice medical director for eligibility decisions and recertification.
  • Collaborate with the interdisciplinary team to review and revise the plan of care.
  • Ensure symptom control through teamwork and adherence to protocols.
  • Perform all other duties as assigned.

Qualifications
  • On an Average Day You Will:
  • (includes, but not limited to) Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
  • Complete history and physical examinations by evaluating the patient\'s medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient\'s medical record
  • Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services
  • Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations
  • Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification
  • Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
  • Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
  • Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
  • Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
  • Consult the collaborating physician when the patient\'s plan of care is outside standardized practice and protocols
  • Collaborate with the interdisciplinary group to create, review and revise the patient\'s plan of care
  • Fulfill the obligation of requested and/or accepted assignments
  • Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
  • Complete all other duties as assigned

Requirements:

  • Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
  • Must have current nursing licensure in State
  • Must have a master\'s degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
  • Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
  • Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
  • Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
