Job summary

Bristol Hospice - Rogue Valley, Grants Pass, OR is seeking a compassionate Nurse Practitioner to provide hospice care to adults and children under the supervision of a collaborating physician. You will apply advanced nursing skills to relieve suffering and support families.

Join an interdisciplinary team dedicated to patient-centered care, comfort, dignity, and respect. The schedule is Monday through Friday, 8am–5pm, serving Jackson, Josephine, and Douglas Counties.

Qualifications

• Graduate of an accredited School of Nursing and NP program.
• Current state nursing license.
• Master's degree with at least 1 year NP experience and 1 year hospice/palliative care experience.
• Accreditation from an approved certifying body for advanced practice nursing as required by the state.
• CHPN or geriatrics certification preferred.

Responsibilities

• Provide medical care, palliate and manage terminal illness under physician supervision.
• Perform history and physical exams; order/interpret tests; document findings.
• Develop and adjust individualized care plans; prescribe meds/treatments; coordinate hospice services.
• Complete hospice face-to-face visit requirements for eligibility and recertification.
• Collaborate with hospice medical director on eligibility and certifications.
• Review clinical information with the interdisciplinary team and coordinate patient care.
• Ensure symptom control through interdisciplinary collaboration.
• Develop and revise care protocols based on literature and expert guidance.
• Consult with physician when plans are outside standard practice.

QualificationsOn an Average Day You Will: (includes, but not limited to)

• Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
• Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record
• Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services
• Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations
• Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification
• Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
• Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
• Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
• Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
• Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols
• Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care
• Fulfill the obligation of requested and/or accepted assignments
• Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
• Complete all other duties as assigned

Requirements

• Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
• Must have current nursing licensure in State
• Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
• Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
• Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
• Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
