Job summary

Hospice de la Luz in Albuquerque seeks a dedicated PRN (per diem) Nurse Practitioner to join our palliative care team, delivering face-to-face visits to patients enrolled in our hospice programs in the Albuquerque area.

You will oversee clinical care in collaboration with the IDG, consult with the hospice physician, educate patients and families, and support staff with palliation. Flexible schedule, work-life balance, and competitive compensation offered.

Qualifications

• Current RN license in the state
• APN license (Advanced Practice Nurse)
• Two years APN experience preferred
• Hospice or home health experience preferred

Responsibilities

• Collaborates with IDG on evaluation and palliation
• Assists in IDG development of care plans for quality care
• Educates patient/family on disease, self-care, and end-of-life care
• Performs and documents patient/family visits per plan
• Interacts with physicians regarding palliation with hospice physician
• Provides education to IDG on palliation
• Active participation in IDG with timely, effective communication
• informs physicians of changes in patient needs and outcomes
